Due to business growth we are currently recruiting a care team leader to work along side our carers and office staff.

Please note a Level 3 in health and social care and driving licence are essential.

What your role will entail:-
To conduct Risk Assessments as part of the Individual Needs Assessment and update these as required as part of the individual’s risk management.
To carry out Individual Needs Assessments for new and potential Service Users.
To carry out introductory visits every time a HSCP visits a Service User for the first time.
To ensure all Service User visits are always covered.
To be on-call in case of emergency.
To liaise closely with Care Coordinators regarding recruitment, permanent allocation of HSCP’s.
To identify suitable HSCP’s to cover for any absences, this may be due to sickness or holiday.
To allocate suitable alternative HSCP when an existing HSCP notifies that they are unable to attend a call.
To facilitate the practical induction of all new HSCP.
To provide and facilitate in-field training as and when appropriate.
To identify any additional training needs of HSCP and report accordingly.
To conduct Service User reviews and respond to Individual Review Forms completed by HSCP as soon as possible.
To urgently report any changes in a HSCP or Service User’s schedule to the Care Coordinator.
To monitor, Spot check, supervise and mentor all HSCP’s in the field.
To carry out assessments, evaluate and develop care plans for service users.
To Implement action to meet and maintain Care standards, this includes ordering, collecting and monitoring medication for service users.
To ensure accuracy and completeness of all documentation held in Service Users homes.
To encourage and identify opportunities for HSCP’s continuing professional development.
To ensure that all written reports and administration is up to date.
To ensure all policy and procedures are followed and in line with the 5 KLOES.
